Travel Hospice Palliative Care RN in Spokane, WA

A Travel Hospice Palliative Care Registered Nurse will provide hospice care in a home health and hospice setting on a 13-week contract in Spokane, Washington. The role supports hospice-eligible adolescents, adults, and geriatrics, with an average daily census of 15 to 16 and charting in Epic and HCHB.

• Complete hospice assessments in patient homes
• Support triage calls and clinical decision-making
• Coordinate goals of care with patients and families
• Document visits and care plans in the charting system
• Educate families on symptom management and medication use
• Collaborate with social services, pharmacy, and interpreter resources

• Washington Registered Nurse license
• BLS certification
• Graduation from an accredited nursing program
• 1 year of skilled hospice or home health experience
• Home Care Home Base experience and triage experience
• Intake assessment, altered mental status, goal setting with patient and family, and proper medication disposal knowledge

• Advanced directives and end-of-life counseling
• Pain, nausea, constipation, and cachexia symptom management
• Patient and family education and support
• Pressure injury prevention and Epic charting experience
