Travel Case Management RN in South Carolina

A Travel Case Management Registered Nurse will coordinate care and discharge planning in a hospital case management unit on a 13-week contract in Charleston, South Carolina. The unit has a 1:25-1:30 caseload and uses MediTech, Midas, and CarePort.

• Coordinate care plans and discharge needs
• Review admissions and utilization needs
• Collaborate with physicians and interdisciplinary staff
• Arrange referrals and community resources
• Document care coordination and patient progress
• Support safe transitions of care

• Registered Nurse license in South Carolina
• 1+ years of acute hospital case management experience
• Core care coordination, utilization review, and discharge planning skills

• Travel experience
• CCM or ACM certification
