Travel Case Management RN in San Antonio, TX

A Travel RN Case Management Registered Nurse will coordinate and facilitate patient care in a long-term acute care hospital on a 13-week contract in San Antonio, Texas. The role supports utilization review, discharge planning, and continuity of care through interdisciplinary collaboration and ongoing assessment.

• Coordinate care transitions with the interdisciplinary team
• Assess patient needs and update care plans across the continuum
• Support utilization review and resource management decisions
• Implement discharge plans and monitor progress toward goals
• Communicate with providers and care teams to promote continuity
• Evaluate outcomes and help improve patient satisfaction and efficiency

• Current Texas Registered Nurse license or multistate Registered Nurse license
• Basic Life Support certification through the American Heart Association
• 2 years of recent RN Case Management experience
• LTAC or STACH experience
• Experience with MCG, discharge planning across acute care settings, and interdisciplinary collaboration
