A Travel Case Management Registered Nurse will manage patient care in the patient's residence on a 13-week contract in Roanoke, Virginia. It supports hospice patients and families with symptom management, crisis intervention, and education. The role works closely with an interdisciplinary team.

• Assess patient needs and changes in condition
• Manage symptom control and crisis intervention
• Educate patients and families on care plans and support needs
• Coordinate care with the interdisciplinary team
• Develop and maintain rapport with patients and families
• Document patient care and communication accurately

• Basic Life Support certification from the American Heart Association
• Virginia Registered Nurse license
