Travel Case Management RN in Pittsfield, MA

A Travel Case Management Registered Nurse will coordinate patient care and utilization management in an acute care hospital for a 13-week contract in Pittsfield, Massachusetts. The role reviews patients at admission and throughout the stay, supports transition planning, and works with a multidisciplinary team. Strong computer, data, and communication skills matter.

• Coordinate patient care needs across the continuum
• Review admissions and ongoing stays for appropriate level of care
• Develop transition plans and post-discharge arrangements
• Collaborate with patients, families, providers, and community resources
• Monitor resource use and support performance improvement efforts
• Prepare summaries and reports for clinical and regulatory needs

• Massachusetts Registered Nurse license
• BLS certification
• Computer literacy and ability to manage multiple open applications
• Knowledge of local community resources
• Organizational, communication, and data analysis skills
• Ability to work independently, maintain confidentiality, and collaborate effectively
