Travel Case Management RN in Nashville, TN

A Travel Case Management Registered Nurse will coordinate discharge planning and utilization review in a hospital case management unit on a 16-week contract in Nashville, Tennessee. The unit uses Midas, Navihealth, and Meditech for patient review and documentation. Caseloads run about 1:30-1:35.

• Coordinate discharge planning and care transitions
• Review patient cases for appropriate level of care
• Document utilization and case management activities
• Collaborate with nurses, providers, and social services
• Monitor lengths of stay and barriers to discharge
• Support safe placement and post-acute referrals

• Active Tennessee Registered Nurse license
• Acute care case management experience
• Previous travel acute care case management experience
• Midas experience
• Meditech experience
• Ability to adapt quickly
