Travel Case Management RN in Englewood, CO

A Travel Case Management Registered Nurse will support discharge planning in an acute care hospital on a 13-week contract in Englewood, Colorado. The unit manages about 20 beds with a case management and social work team and uses Meditech and CarePort.

• Complete discharge planning assessments
• Develop and update discharge plans
• Coordinate transitions of care with the team
• Communicate needs with patients, families, and providers
• Document case management activity in the EMR
• Support complex discharge needs and placement planning

• Colorado Registered Nurse license
• 1 year of current Case Management Registered Nurse experience
• Discharge planning and care coordination skills
• Professional communication and adaptability

• Emergency department experience
• Palliative care knowledge
• Goals of care conversation experience
