Travel Physical Therapist in Lewistown, MT

A Travel Home Health Physical Therapist will provide skilled care in patients' homes and related settings on a 13-week contract in Lewistown, Montana. The role serves home health and hospice patients, with documentation, treatment planning, and caregiver education central to the assignment. Therapy includes mobility, balance, pain management, and safety assessment in the home.

• Review physician therapy orders for home health and hospice patients
• Complete evaluations and develop written treatment plans with goals
• Provide therapeutic exercise, stretching, mobility, and balance interventions
• Assess home safety and recommend equipment or environmental changes
• Monitor progress and update plans to support patient outcomes
• Educate patients, caregivers, and family members on home exercise and safety
• Document visits, progress notes, and discharge paperwork accurately

• Current Montana Physical Therapist license
• BLS certification
• Valid driver's license
• Minimum 2 years of physical therapy experience
