Travel Occupational Therapist in Haverhill, MA

A Travel Occupational Therapist will provide rehabilitation services in a nursing home for a 13-week contract in Haverhill, Massachusetts. The role supports patients with cognitive and ADL needs, uses standardized assessments, and documents care in an electronic record system.

• Screen and evaluate patients with cognitive and ADL limitations
• Develop and adjust plans of care based on assessment and progress
• Provide skilled treatment using ADL retraining, therapeutic activity, exercise, modalities, and assistive devices
• Document treatment, outcomes, and discharge summaries in the electronic record
• Communicate patient progress, barriers, and plans with the care team
• Consult with patients, families, and caregivers, and support assistants or students

• Massachusetts Occupational Therapist license
• Strong skills in patient evaluation, treatment planning, and electronic documentation
